Admission to NHH

A patient needs to meet certain eligibility criteria for NHH assistance, then here are several ways for a patient to enter our Hospice program.

The patient can contact NHH and refer themselves directly.

Family members, clergy, friends, and others who care about a loved one living with a terminal illness may also call NHH at any time to begin the referral process. The patient's physician is often the first to call NHH after consulting with the patient, but really anyone may call or stop by the office if they think hospice might be an option.

The patient's physician does need to supply the formal referral to NHH. After consulting with the patient and the family, the physician can simply call and speak to a hospice nurse. The nurse will then promptly meet with the patient to establish a plan for hospice care.
