Case Management
for the Elderly
Through
Public Health Nursing, we help representatives of healthcare
agencies design interdisciplinary care plans to Big Island
frail elderly at monthly case management team meetings.
The Emergency Bereavement Support
Team
With the Visitors Aloha Society of Hawaii (VASH),
North Hawaii Community Hospital, Hospice of Kona, and Kona
Hospital, we assist visitors to the Big Island who experience
a sudden illness or death while here.
